Section 3: Begin the Application

Personal Information

The applicant must complete each item on this form to be considered for admissions to the Paramedicine Certificate program.

Admissions Checklist:
Evaluation:

Applicants seeking admission to health profession programs will be evaluated on an individual basis. Students seeking admission will be evaluated by their GPA and the total number of college-level credits completed at MassBay. Priority for admission is given to current MassBay students.

The last possible date for admission is January 4th, 2027.

Section 4: Acceptance & Program Expectations

Required Pre-Entry Documentation: After being admitted into a Health Sciences program, students will receive instructions on how to upload required health documentation to an online platform. Students must submit all documents by the published deadline to be eligible to start Health Sciences classes.

  • Medical clearance and proof of immunizations as required by state law and outlined below and in the Division of Health Sciences handbook and in admission paperwork and can be found on the Division of Health Sciences web pages of the MassBay website by clicking the link, Health and Background Check Requirements.
  • Documentation from a healthcare provider of a positive Hepatitis B surface antibody titer, or of a “non-responder” status. A minimum of 6 months is necessary to complete this process.
  • The student must complete the CORI /SORI (Criminal/Sex Offender Record Information) to authorize a search of conviction and pending criminal case information under Standard Required Level 1 by DCJIS (Department of Criminal Justice Information Services). The CORI/SORI completion process is part of the New Student Orientation. If there is a finding of criminal convictions or “open cases”, the student will be ineligible to participate in the Health Sciences Program. A National County Criminal Background check will be conducted as part of the students’ completion of the clinical requirements.
Accommodation for Students with Disabilities

Accessibility Resources Center

MassBay provides equal access for each student who self-discloses a disability and requests accommodations through the Accessibility Resources Center. To initiate an accommodation request, complete the Accommodation Request Form.  Please contact The Accessibility Resources Center with any questions.

Required Immunizations/Titers for Students

After admission, students are required to demonstrate compliance with immunization and immunity verification standards. Evidence of immune status is mandatory for all students prior to enrollment in any Health Sciences courses. These requirements are in accordance with Massachusetts General Laws, Chapter 79, §15D, and the Massachusetts Department of Public Health regulations (105 CMR 220.700).

Required Immunizations and Immunity Documentation
RequirementDocumentation Needed
Tdap1 dose; and a history of a DTaP primary series or age-appropriate catch-up vaccination; Tdap given at ≥ 7 years may be counted, but a dose at age 11-12 is recommended if Tdap was given earlier as part of a catch-up schedule; TD or Tdap should be given if it has been ≥ 10 years since Tdap.
Hepatitis BDocumentation from a healthcare provider of a positive antibody Hepatitis B titer, or of a “non-responder” status. A minimum of 6 months is necessary to complete this process.
MMR2 doses; first dose must be given on or after the 1st birthday, and second dose must be given ≥ 28 days after first dose; laboratory evidence of immunity acceptable; birth in the U.S. before 1957 acceptable only for non-health science students.
Varicella2 doses; first dose must be given on or after the 1st birthday, and second dose must be given ≥ 28 days after first dose; reliable history of chickenpox or laboratory evidence of immunity acceptable; birth in the U.S. before 1980 acceptable only for non-health science students.
Meningococcal1 dose; 1 dose MenACWY (formerly MCV4) required for all full-time students age 21 years of age or younger; the dose of MenACWY vaccine must have been received on or after the student's 16th birthday; doses received at younger ages do not count towards this requirement. Meningococcal conjugate vaccine, MenACWY (MCV4) and MenABCWY, fulfill this requirement; monovalent meningococcal B (MenB) vaccine is not required and does not meet this requirement. Students may decline MenACWY vaccine after they have read and signed the MDPH Meningococcal Information and Waiver Form provided by their institution.
PolioStrongly advised but not required: Documented completed series of polio immunizations. In the absence of polio documentation, vaccination is assumed for students who attended school in the U.S. Students educated outside the U.S. may be at risk unless vaccinated.

Immunity must be demonstrated through blood tests, also called titers, for some conditions listed below. A copy of the complete laboratory report is required for each titer. Physician documentation that says “positive titer” is not adequate.

Clinical Requirements

Students are required to demonstrate compliance with all immunization and health requirements by the established due dates to be eligible for clinical placement and to meet the obligations of legal contracts with affiliating clinical agencies. Students who do not meet these requirements will be prohibited from attending clinicals and, as a result, will be unable to meet the course requirements.

  • Proof of CPR certification. The BLS Provider Card from the American Heart Association, the American Red Cross, or the equivalent from the Military Training Network meets this requirement (Copies of both sides and a valid signature or e-card required).

  • The student must complete the CORI /SORI (Criminal/Sex Offender Record Information) to authorize a search of conviction and pending criminal case information under Standard Required Level 1 by DCJIS (Department of Criminal Justice Information Services) and a National County Criminal Background as part of the clinical requirements. These checks are run by the College at no cost to the student. Clearing the College’s criminal history check does not guarantee that the student will clear licensure agency checks.

  • Flu Vaccine: Annual flu vaccine from the current flu season. The flu vaccine is updated annually*

  • TB (Tuberculosis) Screening: 2-step TB test (two negative PPD tests, taken 1-3 weeks apart) or the QuantiFERON®-TB Gold Plus (QFT-Plus) or the T-SPOT®.TB test (T-Spot) within the past 12 months are acceptable. If you are known to have a positive PPD or blood test prior, you will need a chest x-ray performed within one year prior to admission. You will also need to complete the symptom form. You may be asked to have additional TB testing if a clinical facility requires more recent TB testing prior to starting a clinical rotation. TB Testing is required annually.  Not required for EMT students.

  • COVID Bivalent Vaccination given after 2022*

  • Documentation from a healthcare provider of a complete 3-dose or 2-dose vaccine series or a positive Hepatitis B surface antibody titer or of a “non-responder” status. A minimum of 6 months is necessary to complete this process.

  • Varicella vaccine or titer.

  • Signed Technical Performance Standards form

  • Fingerprinting and drug testing may be required in certain semesters.

*The student may sign a declination form for flu and COVID vaccines. This may impact the student’s ability to have a clinical placement and thus remain in the program.

Please note that these requirements are subject to change based on clinical affiliation requirements. Students are responsible for the cost of meeting these requirements.

Viewpoint Screening

If you are accepted into the program, you will be required to create an account on Viewpoint Screening, an online document storage and tracking system, which is where you will maintain all your health and immunization records.  There is no cost to the student for the Viewpoint Health Portal and Background Check. Viewpoint will send you email “alerts” when documentation is missing, incomplete, overdue, or needs updates.  Students are responsible for checking their Viewpoint accounts regularly and updating health and CPR documentation before it expires.  Authorized Division of Health Sciences staff refer to Viewpoint data to determine whether a student is compliant with all requirements and may attend clinical.

MassBay’s Division of Health Sciences does not accept, review, or maintain student health records. Additionally, submission of medical records to MassBay’s Office of Student Development as a General Studies – Health Track student does not fulfill requirements for Health Sciences programs.
